Establishing new service for your residence or commercial location requires just a few short forms to be filled out and returned to us for processing. The first form is the formal Application for new Services. This form outlines the obligations and conditions of both parties and then becomes the binding agreement after being signed and accepted by us.

The second form we need to receive in order to establish a new account is your preferred payment election. To pay via automatic draft from your bank account please fill out and return the ACH Draft Payment Form. To pay by personal check each month please fill out our Email Invoicing Form.
